We are seeking a skilled Physical Therapist to join our team at Elite Home Health. As a Physical Therapist, you will play a vital role in helping patients recover from injuries or illnesses and regain their independence.
Key Responsibilities- Evaluate patients' functional status and physical therapy needs, working closely with physicians to develop personalized treatment plans.
- Develop and implement effective treatment plans to improve patients' mobility, endurance, and overall well-being.
- Monitor patients' progress, reporting any changes or concerns to the supervising nurse and/or physician.
- Collaborate with the healthcare team to ensure seamless care and communication with patients and their families.
- Current Physical Therapy licensure in the state of practice.
- Current CPR certification.
- A valid driver's license and vehicle insurance, with access to a reliable vehicle.
