We are seeking a skilled RN Infusion Liaison to provide direct nursing services related to infusion therapy for patients in various settings. The primary focus of this position will be to provide education to patients discharging from area hospitals, while secondary responsibilities will involve providing infusion nursing care in the home or clinic.
Key Responsibilities- Provide education to patients discharging from area hospitals on infusion therapy and related care.
- Collaborate with various providers, patients, caregivers, physicians, and payors to ensure seamless care coordination.
- Provide infusion nursing care in the home or clinic, as needed.
- Develop and maintain relationships with patients, families, and healthcare teams to ensure comprehensive care.
- Stay up-to-date with the latest infusion therapy guidelines, best practices, and regulatory requirements.
- Current Oregon Registered Nurse License or Washington Registered Nurse License (Vendor Managed) or National Registered Nurse Compact License.
- National Provider BLS - American Heart Association certification within 30 days of hire.
- Unencumbered Registered Nurse License in state(s) of hire and wherever care is delivered.
- 1 year of experience in acute hospital care, infusion, or home health.
