Healthcare Professional

Optum Home and Community Care is seeking a skilled Healthcare Professional to join our team as a Home-Based Medical Care Specialist. As a key member of our interdisciplinary team, you will work closely with patients, caregivers, and healthcare providers to deliver high-quality, patient-centered care in the comfort of the patient's own home.

• Conduct comprehensive assessments of patients to identify their unique needs and develop personalized care plans.
• Collaborate with patients, caregivers, and healthcare providers to ensure seamless care coordination and communication.
• Provide education and support to patients and caregivers on disease management, wellness, and self-care strategies.
• Participate in ongoing quality improvement initiatives to enhance patient outcomes and experience.
• Stay up-to-date on relevant medical and nursing research, technology, and best practices to provide evidence-based care.

• Completion of a Nurse Practitioner accredited program.
• Current NP license and state RN license.
• Knowledge of long-term care, community resources, and cost-effective alternatives.
• Access to reliable transportation.

• 2+ years of clinical experience in primary care, home-based medical care, geriatrics, or palliative care.
• Knowledge of CPT, ICD-10, and HCPC codes.
• Awareness of NCQA requirements, CMS guidelines, Medicaid/Medicare contracts, and benefit systems.
