We are seeking a dedicated and compassionate Physical Therapist to join our award-winning team.

Job Summary:

The Physical Therapist will provide direct rehab care in physical therapy to patients upon physician referral in accordance with all applicable laws, regulations, and Life Care standards.

Responsibilities:
  • Develop and implement individualized physical therapy treatment plans for patients
  • Conduct thorough assessments and evaluations to identify patient needs and goals
  • Collaborate with interdisciplinary teams to ensure comprehensive care
  • Provide education and training to patients and their families on physical therapy techniques and exercises
  • Maintain accurate and timely documentation of patient progress and treatment plans
  • Participate in quality improvement initiatives and contribute to a culture of excellence
Requirements:
  • Graduate of an accredited program in physical therapy, BSPT, MSPT, or DPT (2003 or later must have MSPT or DPT)
  • Current registration/licensure in applicable State
  • One (1) year experience in post-acute care or related setting preferred
  • CPR certification upon hire or obtain during orientation
