Healthcare Advocate Coordinator

The Key Responsibilities:
As a Healthcare Advocate Coordinator, you will be the face of Senior Doc at our partner senior living communities, providing expert care and support to residents, staff, families, and healthcare providers. Your role will involve taking vitals and weights on community residents monthly, coordinating and communicating care services and health needs for residents, facilitating telehealth appointments, and conducting wellness checks for patients enrolled in our programs.

What You'll Do:
• Provide expert care and support to residents, staff, families, and healthcare providers.
• Ensure smooth, efficient coordination of care between healthcare providers and residents.
• Develop and implement care plans to improve patient outcomes and enhance overall quality of life.
• Participate in community and company events, promoting health and wellness among seniors.
• Educate and support families on how to manage conditions, teach self-care techniques, and provide resources for better health.
• Collaborate with healthcare providers to identify and address any gaps in care.
• Assist in quality improvement initiatives to enhance the quality of life for our patients.

Requirements:
• Medical Assistant Degree required.
• High school diploma, GED, or equivalent.
• Experience with geriatrics must have experience working with the senior population.
• Experience or education in patient care and/or case management.
• A professional, outgoing personality with a strong sense of initiative.
• Ability to work independently and thrive in a dynamic environment.
• Willingness to travel daily to assigned locations and attend community events outside the typical Monday-Friday, 8-5 schedule.
• Ability to stand for extended periods.
• Phlebotomist License (preferred but not required).
