Terros Health is seeking a dedicated and compassionate Case Manager to join our team at our 51st Ave Healthcare Clinic in Phoenix, AZ. As a key member of our interdisciplinary clinical team, you will work with individuals diagnosed with Serious Mental Illness or General Mental Health diagnosis under Court Ordered Treatment. Your primary responsibility will be to assist clients in identifying treatment and recovery goals, and provide supportive services to help them reach their objectives. You will also complete assessments, crisis plans, and individualized service plans, and implement services identified. Effective communication and documentation of client progress toward their recovery are essential. We value a recovery philosophy and promote a safe recovery environment for our clients. You will use brief clinical interventions, Motivational Interviewing, and supportive listening skills to engage and support clients receiving services. Additionally, you will engage and provide direct supports to clients' natural support systems. Collaboration with external service providers, including medical providers, is crucial to ensure best care is provided to our clients. You will work under the direct supervision of our team Clinical Coordinator.

Key Responsibilities:

* Assist clients in identifying treatment and recovery goals and provide supportive services to help them reach their objectives

* Complete assessments, crisis plans, and individualized service plans

* Work collaboratively with the clinical team to engage, educate, communicate, and coordinate care with members, their families, behavioral health, medical, and dental providers, community resources, and others to ensure that all services prescribed in the individualized service plan are implemented

* Conduct outreach and follow-up of services, including crisis interaction, missed appointments, and hospital discharges, to ensure adequate resources are available and in place

* Perform all case management functions associated with caseload, including completing the assessment and service planning processes, identifying the need for further or specialty evaluations, and pursuing best practice outcomes for individuals with mental illness, including continuing education, employment, independent housing, and community tenure

Qualifications:

* Education/Experience: GED/High school diploma with a minimum of 1.5 years of behavioral health experience, or an associate's degree in the field of medical or behavioral health, or a bachelor's degree (not in the field of behavioral health) and 1 year of behavioral health experience, or a bachelor's degree in the field of behavioral, or a master's degree or higher in the field of behavioral health

* Computer skills: Microsoft Word, Excel, Outlook, electronic medical record, NextGen preferred

* Valid AZ driver's license, be 21 years of age with minimum 4 years driving experience, and meet requirements of Terros Health's driving policy

* Must have a valid Arizona Fingerprint Clearance card (Level 1) or apply for an Arizona Fingerprint Clearance card within 7 working days of employment

* Interpersonal skills that allow for professional and positive relationships with co-workers, members, families, and other community members

* Good communication skills, evidenced by a non-judgmental approach, effective listening, and positive interactions

* Basic understanding of the DSM-V preferred

* Certified Case Manager strongly preferred

* Must pass background check, TB test, and other pre-employment screening
