BAYADA Home Health Care is seeking a skilled Licensed Practical Nurse (LPN) to join our team as a Home Health Care professional. As a LPN, you will be responsible for providing high-quality patient care in the comfort of clients' homes.
Key Responsibilities- Conduct home visits to assess clients' needs and provide personalized care.
- Administer medications, perform wound care, and provide treatments as prescribed by physicians.
- Monitor clients' conditions and report any changes to the Clinical Manager and Client Services Manager.
- Follow up with physicians to execute and document their orders.
- Perform client assessments as necessary and provide case management and coordination.
- Accurately document observations, interventions, and evaluations in electronic medical records.
- A current license as a Licensed Practical Nurse in the state of New Jersey.
- A minimum of one year of recent, verifiable experience as an LPN.
- Graduation from an accredited and approved nursing program.
- Prior home care experience is strongly preferred, but not required.
- Demonstrated ability to read, write, and effectively communicate in English.
- Ability to work independently and manage time effectively.
- Strong interpersonal skills.
- Solid computer skills, with prior experience with electronic medical records (EMR) preferred.
