Job Summary:

As a Registered Nurse - Person Served Care Coordinator, you will play a vital role in ensuring the comprehensive care of our rehabilitation team members. Under the direction of the Director of Nursing, you will collaborate with other team members to provide exceptional care and support to our patients.

Key Responsibilities:

Collaborate with rehabilitation team members to ensure comprehensive care of person served
Collaborate with mid-level provider and medical director on changes in condition of person served
Assist in education, monitoring and coaching the nursing staff on continuous quality improvement measures, and assist in implementing nursing policies and procedures
Arrange transportation to follow up external doctor appointments, testing, etc.
Communicate with person served family on continued changes or updates to plan of care
Assist with discharge planning including family education and training
Collaborate with other members of the nursing leadership team to provide support and coverage for other leaders as needed

Requirements:

Current licensure in good standing to practice as a Registered Nurse in the State of Iowa
At least 2 years of nursing experience
Ability to maintain CPR certification
Excellent communication and interpersonal skills
Ability to work in a fast-paced environment and prioritize multiple tasks
