Job Responsibilities:
- Provides coordinates and directs the provision of home nursing care according to physicians orders based on agency policies and procedures through the competent application of the nursing process.
- Responsible to Clinical Manager Directly Supervises and Evaluates Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses.
Travel Details:
- Home visits.
- Periodic office meetings.
- Must have reliable transportation, current drivers license, and agency-required liability insurance.
Productivity Expectations:
- Average # of patients/day: 5-8.
- Points per day/week: How are points broken down 30 points per week. Each type of visit and activity (documentation travel etc.) hold a different point value.
Contract Details:
- Guaranteed Hours: No if there is a situation where the contractor is unable to meet their financial needs due to low patient census please reach out to the AM to discuss floating the contractor to a nearby clinic to pick up hours.
- Types of Cases: Start of Care Wound Care Follow up Visits etc.
- Years of experience REQ: Minimum 1 full year Adult Home Health Experience or 1 full year Hospice experience depending on the specialty.
- First-timers accepted: (Y/N) No must have recent specialty experience.
- Weekend REQ: Varies per location and profession please have traveler discuss during the interview.
- Certs REQ: CPR BLS.
- Is on-call REQ (Y/N) What is the req Varies per location and profession please have traveler discuss during the interview.
- Territory/County covering: Territory mileage radius Must be willing to travel up to 75 miles from home clinic.
- Mileage reimbursed IRS Standard rate. Traveler is compensated from patient home to patient home. The commute to and from the first and last patient are not billable.
- Locals accepted: (Y/N) What is the local radius rule Yes local accepted at the same rate.
- Pending License accepted: (Y/N) No must have license in hand.
