We are seeking a highly skilled and experienced MDS Coordinator to join our team at Brookdale Senior Living. As a key member of our clinical staff, you will be responsible for conducting and coordinating the development and completion of resident assessments, as well as overseeing the generation of Minimum Data Set (MDS) for each Medicare patient.
Key Responsibilities- Conduct and coordinate the development and completion of resident assessments in accordance with State, Federal, and Company guidelines.
- Oversee the generation of Minimum Data Set (MDS) for each Medicare patient and ensure timely electronic transmission of required data.
- Coordinate the development and completion of the resident assessment (MDS) in accordance with current regulations and guidelines.
- Develop and implement procedures for the arrival of newly admitted residents and ensure timely completion and transmission of assessments.
- Assist community directors and supervisors in scheduling resident assessment/care plan meetings and coordinate participation by outside members of the care plan team.
- Develop and maintain written policies and procedures governing the development, use, and implementation of the resident assessment (MDS) and care plan.
- Develop and implement an ongoing quality assurance program for resident assessment/care plans and monitor community QI and QM reports.
- Assist with discharge planning and participate in community surveys and inspections.
- Provide leadership and participate in various committees, including Interdisciplinary Care Plan Team, Policy Advisory, and Quality Assessment and Assurance.
- Nursing degree from an accredited college or university.
- Minimum of two (2) years of experience as a supervisor in a hospital, nursing care community, or other related health care facility.
- Minimum of six (6) months training experience in rehabilitative and restorative nursing practices.
- Current State LPN/LVN license.
This position requires interaction with co-workers, residents, and vendors. The employee may be exposed to communicable diseases and infections. The ability to lift up to 50 pounds is required.
Salary and BenefitsThe salary for this position is $31.63 per hour. Brookdale offers a comprehensive benefits package, including health insurance, life insurance, and retirement plans.
