Job Summary

Premise Health is a leading direct healthcare provider seeking a skilled Part-Time Licensed Practical Nurse (LPN/LVN) to join our team in an ambulatory care setting. As a key member of our healthcare team, you will provide high-quality patient care and services in a fast-paced environment.

Key Responsibilities
  • Conduct patient interviews to determine medical problems/conditions and document in patient charts for physician review.
  • Obtain and record patient vital signs and weight.
  • Assist physicians with patient examinations as needed.
  • Review physician orders, lab requests, and follow-up needs with patients.
  • Document services performed on appropriate forms.
  • Dress wounds as instructed and apply compresses, ice, and heat packs.
  • Observe patients and report adverse reactions to medication or treatment to medical personnel in charge.
  • Administer specified medication, orally or by subcutaneous or intermuscular injection, and note time and amount on patient charts.
  • Assemble and use clinic equipment.
  • Collect samples, such as urine, blood, and sputum, from patients for testing and perform routine laboratory tests on samples.
  • Sterilize equipment and supplies.
  • Clean rooms and change linens.
  • Maintain supplies, including stocking and ordering new supplies and disposing of expired or damaged supplies.
  • Respond to patient or client telephone inquiries by evaluating and responding to patient/client problems and questions.
  • Follow up with patients who miss appointments.
  • Follow up with patients on lab results, including scheduling any additional tests.
  • Coordinate referrals for patients through insurance and other physician offices.
Requirements
  • Graduate of a National League for Nursing accredited program, Associate's degree (A.S.) and/or equivalent from a two-year college or technical school.
  • Licensed as a Practical or Vocational Nurse.
  • Current hands-on Certification in Basic Life Support is required, Advanced Cardiac Life Support may also be required based on scope of contract services.
  • At least three years of clinical experience as a LPN/LVN in an ambulatory care, occupational health, or emergency department setting.
Preferred Qualifications
  • Knowledge about workplace health and safety concepts and OSHA regulations.
  • Strong computer skills (Internet software, spreadsheet, word processing, etc.) preferred.
  • Knowledge and experience with Electronic Medical Records preferred.
