Job Summary:
We are seeking a skilled Licensed Practical/Vocational Nurse (LPN) to join our team at Ascension, working night shifts in a Medical/Surgical Specialty setting. As a Senior Care Nurse, you will provide high-quality patient care under the direction of a registered nurse or physician, functioning within the scope of your license.
Key Responsibilities:
- Contribute to the assessment of patients and ensure their well-being.
- Provide education to patients and families about disease treatment plans, including self-care post-discharge, holistic health needs, available resources, and follow-up care.
- Collaborate with the healthcare team to develop and implement individualized care plans.
- Participate in quality improvement initiatives to enhance patient outcomes and satisfaction.
Requirements:
- Licensed Practical/Vocational Nurse credentialed from the Michigan Board of Nursing obtained prior to hire date or job transfer date.
- Ability to work night shifts, 7PM-7:30AM.
- Strong communication and interpersonal skills.
- Ability to work effectively in a fast-paced environment.
