Community Rehabilitation Worker

">About the Role:

We are seeking a highly motivated and energetic Community Rehabilitation Worker to join our team at the Institute for Comm Living. As a Community Rehabilitation Worker, you will work closely with consumers in independent apartments in the community, providing support and assistance to help them achieve their goals and maintain a high level of independence.

">Responsibilities:

The successful candidate will be responsible for:

  • Providing supportive counseling to consumers in 1:1 or group formats to assist them in modifying attitudes and behaviors as needed;
  • Securing resource information for consumers in all aspects of their care and disseminating it to them in a clear and understandable manner;
  • Maintaining up-to-date, accurate individual case records on each assigned consumer and developing measurable and objective service plans that maximize consumer rehabilitative abilities;
  • Facilitating individualized services to the consumer that meet their diverse needs and focus on discharge-planning goals;
  • Assisting consumers in finding and getting into vocational training or other training opportunities.
">Requirements:

To be successful in this role, you will need:

  • A bachelor's degree plus one year of experience providing direct services to individuals with mental disabilities; or an associate's degree in a human services-related field and two years of experience in human services;
  • Ability to work with consumers, families, and staff in a caring and respectful manner, with due understanding and consideration for cultural differences;
  • Effective communication and interpersonal skills, with the ability to use sound judgment in identifying and solving problems;
  • Ability to organize time effectively to accomplish all tasks in a timely way and meet deadlines without prompting;
  • Willingness to continually learn and apply knowledge and participate in in-service training and development activities.
