About the Role:

We are seeking a highly skilled Patient Care Coordinator to join our pharmacy team at Premier Community Healthcare Grp.

This role involves working closely with pharmacists and other healthcare professionals to provide personalized patient care services, ensuring seamless coordination and efficient communication.

Key Responsibilities:

  • Collaborate with pharmacists to develop and implement patient care plans, addressing medical, social, and emotional needs.
  • Communicate effectively with patients, families, and healthcare providers, fostering trust and understanding.
  • Maintain accurate records, track progress, and identify areas for improvement.
  • Ensure compliance with regulatory requirements, company policies, and industry standards.

Requirements:

  • Florida Board of Pharmacy Registered Pharmacy Technician certification
  • High School diploma or equivalent
  • Minimum 1-2 years of experience as a registered pharmacy technician
  • Prior experience with Epic certification preferred

Working Conditions:

  1. Able to lift up to 20 lbs regularly and 30-50 lbs occasionally
  2. Ability to sit for long periods and perform repetitive tasks
  3. Direct exposure to computer screens

Estimated Salary: $45,000 - $55,000 per year, based on experience and qualifications.
