  • Supervise and manage patient care episodes from admission to discharge, ensuring seamless transitions and optimal patient outcomes.
  • Coordinate patient care with other clinical staff and medical providers, fostering effective communication and collaboration.
  • Perform home health admission visits, resumptions, recertifications, and discharges, providing comprehensive care and support to patients and their families.
  • Record, assess, and observe patient progress, reactions, and symptoms, using data-driven insights to inform care decisions.
  • Assess patient needs and develop personalized plans of care, ensuring alignment with agency, state, and federal guidelines.
  • Check vital signs, administer medication, and provide wound care, ensuring patients receive timely and effective interventions.
  • Educate patients and their families on disease management, treatment options, and self-care strategies, promoting empowerment and independence.
  • Document care in patient records, adhering to agency, state, and federal regulations and guidelines.
  • Provide home care to patients with acute health conditions, injuries, or illnesses, or those requiring pharmaceutical interventions.

Requirements:

  • Graduation from an accredited nursing school and a current, valid RN license registered in the state of Texas.
  • OASIS training and certification.
  • Valid driver's license and willingness to travel throughout assigned territory.
  • At least one year of home health nursing experience, with a proven track record of delivering high-quality patient care.
