Role Overview

Moments Hospice is seeking a compassionate Certified Nursing Assistant (CNA) to join our dedicated team of caregivers. As a CNA at Moments Hospice, you will play a vital role in providing personalized care and support to patients nearing the end of life.

Responsibilities
  • Provide Personalized Care: Perform personal care activities related to daily living, such as bathing, dressing, grooming, toileting, skin care, and more.
  • Review Patient Care Plans: Collaborate with our RN Case Manager to review patient care plans and adjust as needed.
  • Lift and Transfer Patients: Assist with lifting and transferring patients safely and efficiently.
Requirements
  • Certified Nursing Assistant (CNA) License: Current CNA license required.
  • 12 Months of Caregiving Experience: Minimum 12 months of caregiving experience in Home Care, Assisted Living/Memory Care, Skilled Nursing/Nursing Home, Hospital, or Hospice settings.
  • Excellent Customer Service Skills: Strong communication and customer service skills are essential.
