Your Role
As an Acuity Care Nurse Practitioner, you will be responsible for:
- Providing comprehensive nursing care to patients, including assessment, planning, intervention, and evaluation.
- Developing and implementing individualized care plans in collaboration with patients, families, and interdisciplinary teams.
- Communicating effectively with physicians, nurses, and other healthcare professionals to ensure seamless care coordination.
- Participating in discharge planning and ensuring a smooth transition for patients to their next level of care.
Qualifications
To be considered for this role, you must have:
- Current state licensure as Registered Nurse.
- BCLS certification. ACLS preferred.
- Minimum three years' experience in a Subacute or ICU setting.
