Job Description:

We are looking for a highly skilled Ambulatory Care Pharmacist - Clinical Specialist to play a crucial part in our team's efforts to deliver exceptional pharmaceutical care. As a key member of our ambulatory care team, you will conduct comprehensive medication reviews, develop personalized medication action plans, and collaborate with healthcare providers to optimize medication therapy.

The ideal candidate will possess a strong background in ambulatory care pharmacy, excellent communication skills, and a commitment to patient well-being. Additionally, proficiency in utilizing electronic health record (EHR) systems is required. Board certification in ambulatory care pharmacy (e.g., BCACP) is preferred but not necessary.

In this challenging and rewarding role, you will have the opportunity to work closely with patients, healthcare providers, and other members of the interprofessional team to ensure seamless transitions of care and optimal therapeutic results. You will also participate in quality assurance programs and continuous improvement initiatives to enhance pharmacy services.

Key Responsibilities:
  • Conduct comprehensive medication reviews for ambulatory patients to identify, resolve, and prevent medication-related problems.
  • Develop personalized medication action plans and ensure proper medication management and adherence.
  • Collaborate with healthcare providers to optimize medication therapy and enhance patient outcomes.
  • Provide patient counseling and education on medication use, adherence, and potential side effects.
  • Monitor patient progress and adjust medication therapy as needed to achieve optimal therapeutic results.
  • Utilize electronic health records (EHR) to document interventions and track patient outcomes.
  • Participate in the development and implementation of clinical protocols and guidelines for ambulatory care.
  • Stay current with best practices and advancements in ambulatory care pharmacy.
Preferred Qualifications:
  • Bachelor's of Pharmacy or PharmD degree from an accredited pharmacy program.
  • Experience in ambulatory care pharmacy.
  • State pharmacy license in good standing.
  • Proficiency in utilizing electronic health record (EHR) systems.
  • Strong analytical and problem-solving skills.
  • Excellent communication and collaboration abilities.
Compensation and Benefits:

This full-time position offers a competitive salary range of $120,000-$160,000 annually, depending on experience. Our comprehensive benefits package includes medical, dental, and vision coverage, 401(k) matching, and paid time off.
