The Registered Nurse Case Manager position is a key part of our team.

Job Description

This role involves working closely with patients, families, and healthcare providers to assess, plan, implement, and evaluate patient care plans.

The ideal candidate will have strong communication and interpersonal skills, as well as the ability to work independently and as part of a team.

Key Responsibilities

  1. Conduct thorough assessments of patient needs and develop personalized care plans
  2. Collaborate with healthcare providers and caregivers to ensure effective care delivery
  3. Monitor patient progress and make adjustments to care plans as needed
  4. Communicate effectively with patients, families, and healthcare teams

Requirements and Qualifications

To excel in this role, you will need:

  • A graduate degree in nursing (AD, Diploma, or BSN) from an accredited institution
  • Current licensure to practice professional nursing in North Carolina or South Carolina
  • At least one year of RN experience and a total of two or more years of clinical experience
  • CPR certification
