  • Provide high-quality, patient-centered care in the comfort of their own homes
  • Work collaboratively with our interdisciplinary team to develop and implement personalized care plans
  • Conduct thorough assessments, take vital signs, and document patient progress
  • Assist patients with self-care techniques and educate them on proper medication management
  • Collaborate with our team to coordinate patient care and plan for discharge
Requirements
  • Graduate of an accredited practical nursing program and active LPN license in Kansas
  • CPR certification
  • Knowledge of state and federal home health regulations
  • Clinical proficiency in nursing, strong communication skills, and compassion
