Job Summary

We are seeking a highly skilled and compassionate Home Visit Nurse Practitioner to join our team at One Medical Seniors. As a key member of our high-risk home-based care team, you will provide comprehensive care to geriatric, frail, and clinically complex patients in the Central Valley area of Phoenix, AZ.

Key Responsibilities
  • Conduct thorough initial visits with patients, gathering medical, social, and behavioral context to inform care plans.
  • Work collaboratively with the patient and multidisciplinary care team to impact ongoing care, focusing on preventable hospitalizations and aligning care with patient goals.
  • Manage a panel of approximately 150 case-mix adjusted patients, balancing home visits and telephone/video interventions.
  • Collaborate with primary care teams and specialists to ensure seamless care coordination.
  • Participate in team huddles to review care plans and assess patient goals.
Requirements
  • 2+ years of home-based complex care, in-office complex care, geriatric primary care, geriatrics, or palliative care experience as a Registered Nurse or Nurse Practitioner.
  • 6+ months of NP experience.
  • Current licensure in the state of Arizona.
  • Completed an accredited NP program with a national certification (ANCC, AANP) in Family, Geriatrics, or Adult.
  • Unrestricted driver's license and ability to drive to patient homes.
