Key Responsibilities- Evaluate and Treat Patients: Conduct thorough assessments of patients to identify treatment needs and develop personalized plans to achieve established goals.
- Administer Direct Patient Care: Provide high-quality care to patients in accordance with written prescriptions and referrals from licensed physicians or authorized practitioners.
- Design and Implement Treatment Programs: Create and implement treatment plans that address patient needs and promote optimal outcomes.
- Monitor Patient Progress: Regularly assess patient tolerance and status during treatment and modify treatment plans as needed to ensure optimal results.
- Document Patient Information: Maintain accurate and complete medical records for each patient in accordance with department policies and procedures.
- Participate in Quality Improvement: Collaborate with interdisciplinary staff to participate in quality improvement chart reviews and activities.
- Communicate with Patients and Families: Interact with patients and families to provide education and support, and to address concerns and questions.
- Education: Master's Degree in Physical Therapy required, or equivalent combination of education and related experience.
- Licensure: Current License to practice as a Physical Therapist in New York State required, plus specialized certifications as needed.
- Experience: 0-3 years of relevant experience required.
