About the Role

We are seeking a skilled Nurse Practitioner to join our HouseCalls team, providing high-quality care to patients in the comfort of their own homes. As a key member of our team, you will conduct in-home assessments, perform evidence-based practice screenings, and identify diagnoses to inform care management.

Responsibilities
  • Conduct comprehensive in-home assessments, including evaluations of past medical history, review of systems, medication reconciliation, vital signs, and physical exams.
  • Perform point-of-care testing and evidence-based practice screenings as appropriate.
  • Identify diagnoses and develop care plans to address patient needs.
  • Communicate findings with patients' primary care physicians to ensure seamless care coordination.
  • Educate patients on disease processes, medication management, and compliance.
Requirements
  • Active NP license or ability to obtain by start date.
  • Active ANCC or AANP national certification in Family, Geriatrics, Adult, Adult-Gerontology Primary Care, or Adult-Gerontology Acute Care certification.
  • Ability to complete physical requirements of the job, including lifting a 30-pound bag and navigating various dwelling conditions.
  • Reliable transportation for home visits.
Preferred Qualifications
  • 1+ years of clinical experience in a related field.
  • Experience in gerontology, cardiology, internal medicine, or endocrinology.
  • Home health care or home visit experience.
