At Crossroads Hospice & Palliative Care, we're committed to delivering exceptional care to our patients and their families. As a Registered Nurse Case Manager, you'll play a vital role in providing direct care to patients in their homes, whether it's at home or in a care facility.
Key Responsibilities:- Deliver high-quality patient care, addressing their medical, emotional, and social needs
- Build trusting relationships with patients and their families, serving as a central point of communication
- Collaborate with our interdisciplinary team of holistic care providers to ensure comprehensive care
- Provide education and support to patients and their families, empowering them to make informed decisions
- 6 months minimum prior skilled nursing experience
- Active, unencumbered state Registered Nurse license
- Current driver's license and auto liability coverage
- Interest and ability in providing patient care in homes and facilities
- Monday to Friday, 8:30 AM to 5:00 PM schedule
- No mandatory on-call required
