Job Summary

We are seeking a highly skilled Clinical Pharmacist to join our team at Providence Sacred Heart Medical Center in Spokane, WA. As a dedicated transitions of care pharmacist, you will play a critical role in ensuring the safe and effective transition of patients from hospital to home or other care settings.

Key Responsibilities
  • Conduct admission medication reconciliation for hospitalized patients
  • Oversee technicians and precept students and residents
  • Develop and implement quality improvement initiatives to enhance patient care
  • Collaborate with interdisciplinary teams to optimize patient outcomes
Requirements
  • PharmD or Bachelor's Degree in Pharmacy
  • Washington Pharmacist License and Board of Pharmacy Preceptor license
  • Basic Life Support-Provider (BLS) certification
Preferred Qualifications
  • Acute care hospital pharmacy experience
