About the Role

We are seeking a highly skilled Care Transition Nurse to join our team at CommonSpirit Health at Home. As a Care Transition Nurse, you will play a critical role in ensuring the smooth transition of patients from hospital to home care settings.

Key Responsibilities
  • Assess patient eligibility for home infusion services and provide guidance to discharge coordinators and healthcare providers.
  • Develop and maintain professional relationships with healthcare providers, facilities, and patients to promote home infusion services.
  • Communicate with attending physicians to obtain verbal or written orders for home care services.
  • Assist in ordering equipment and providing patient instructions on procedures and equipment.
  • Participate in educating and in-servicing new providers and personnel on home infusion services.
  • Meet with the Director, Operations and other organizational employees to ensure continuity of care for patients.
Requirements
  • Graduate of an accredited school of nursing.
  • Experience as a Care Transition Nurse or similar role, sales or marketing.
  • Hospital and/or homecare nursing experience.
  • Current RN, or LPN, license to practice in the state serviced.
  • Obtain and maintain current CPR certification; able to perform CPR.
  • Compliant with appropriate state certification/registration and organizational requirements as necessary.
  • Two years of appropriate nursing experience.
  • Knowledge of home care regulations required.
  • Strong assessment, critical thinking, judgment, creativity and decision making skills.
  • Excellent communication skills; both verbal and written.
Compensation

$39.80/hour
