Nurse Practitioner

Job Summary:
We are seeking skilled and compassionate LVN's and RN's to join our team at Gracy Woods I Nursing Center. As a key member of our healthcare team, you will provide high-quality nursing care to our residents, promoting their physical, emotional, and social well-being.
Responsibilities:Essential Duties and Responsibilities:
- Provide individualized nursing care to residents, assessing their needs and developing personalized care plans.
- Administer medications, treatments, and therapies as prescribed by physicians, ensuring accurate documentation and adherence to facility policies.
- Monitor and report changes in resident condition, collaborating with healthcare team members to ensure timely interventions.
- Implement and maintain a safe and clean environment, adhering to infection control and safety protocols.
- Communicate effectively with residents, families, and healthcare team members, promoting a positive and respectful atmosphere.
- Participate in ongoing education and training, staying current with best practices and regulatory requirements.
- Collaborate with interdisciplinary teams to develop and implement care plans, ensuring comprehensive and coordinated care.
- Document resident care and progress, maintaining accurate and detailed records.
- Assist with resident transfers, admissions, and discharges, ensuring a smooth transition of care.
- Perform other duties as assigned, supporting the overall mission and goals of Gracy Woods I Nursing Center.
Education and Experience:
We require a graduate of a basic education program in professional nursing, with a minimum of one year of medical-surgical nursing experience in an acute care setting or long-term care. Critical care experience is preferred.
License and Certification:
A current RN license in the State(s) of practice is required. A valid driver's license and automobile insurance are also necessary.
Working Conditions:
This role requires the ability to move about intermittently during working hours, including standing, lifting, bending, stooping, twisting, pushing, and pulling. The ability to read, write, and speak the English language is essential. The position involves interacting with residents, family members, staff, visitors, government agencies, and other personnel under all conditions and circumstances.
