About the Role:

  • Develop and implement individualized care plans to address patients' physical, emotional, and social needs.
  • Collaborate with multidisciplinary teams to ensure seamless transitions between care settings.

Qualifications:

  • Bachelor's Degree in Nursing or related field.
  • At least two years of nursing experience.
  • Active Registered Nurse (RN) license.

Estimated Salary:** $85,000 - $110,000 per year**, based on your skills and qualifications.
