Key Responsibilities- Plan of Care: Develop and implement individualized occupational therapy plans to meet the unique needs of each patient.
- Evaluation: Conduct thorough evaluations to assess patients' functional abilities and identify areas for improvement.
- Patient Outcomes: Utilize evidence-based practices to improve patients' range of motion, muscle strength, and coordination.
- Minimize Patient Risk: Educate patients and their families on preventative measures to reduce the risk of re-hospitalization.
- Family Educator/Advocate: Provide counseling and education to patients and their families on disease management and proper use of equipment.
- Supervision: Oversee and supervise the care provided by Certified Occupational Therapy Assistants (COTAs).
- Communication/Collaboration: Communicate effectively with the interdisciplinary team to ensure seamless care coordination.
- Policies and Regulations: Adhere to agency policies, procedures, rules, and regulations, and maintain accurate documentation.
- Occupational Therapist License: Current license in the state of employment.
- Experience: Minimum one year of experience as an Occupational Therapist in an acute care setting, with home care experience preferred.
- Driver's License: Valid driver's license and auto insurance in your name as a driver.
- Physical Demands: Ability to meet the physical demands of the job.
