We are a leading provider of primary care and behavioral health services to the U.S. Department of Veterans Affairs, managing and operating ten Veteran-only primary care outpatient clinics across the country. Our mission is to provide high-quality healthcare to America's Veterans, and we are seeking a dedicated Registered Nurse to join our team.
Responsibilities:- Provide patient-centered care as part of a Patient Aligned Care Team (PACT)
- Assist practitioners with patient care and practice specialized nursing skills in patient assessment
- Triage urgent/emergent same-day patients and identify high-acuity patients
- Support patients in navigating and coordinating their care
- Participate in daily team huddles and provide patient and family education
- Monitor discharge lists and contact patients for follow-up
- 1 year of primary care and/or chronic disease management experience
- Current, active, and unrestricted nursing license in the state of practice
- Current BLS for providers from the American Heart Association
- Strong computer skills, including EMR experience
