Key Responsibilities
  • Assessment and Planning: Conduct thorough assessments of patients' physical, emotional, and social needs, and develop comprehensive care plans that address these needs.
  • Case Management: Provide primary care case management to assigned patients, ensuring that their care is coordinated and effective.
  • Communication: Communicate effectively with patients, families, and healthcare teams to ensure that care is delivered in a timely and efficient manner.
  • Documentation: Maintain accurate and detailed documentation of patient care, including progress notes, treatment plans, and other relevant information.
Requirements
  • Education: Associate's Degree in Nursing (ADN) or Bachelor's Degree in Nursing (BSN)
  • Experience: Minimum of 2 years of recent medical/surgical nursing experience, with a preference for home health or hospice experience
  • Certifications: RN - Registered Nurse, CPR - AHA Healthcare Provider, and CHPN - Certified Hospice Palliative Nurse (within 4 years of hire)
Preferred Qualifications
  • Education: Bachelor's Degree in Nursing (BSN)
  • Experience: 2-4 years of recent medical/surgical nursing experience, with a preference for home health or hospice experience
