We have specialty home care programs available, including adult and pediatric asthma, cardiac, diabetes, orthopedic and wound care programs. Our services are designed to meet the unique needs of each patient, and our team works closely with healthcare providers to ensure seamless care coordination.
About the Job
As a Home Care Physical Therapist, you will be responsible for providing direct patient care activities in the home environment. This includes assessment, evaluation, treatment planning, implementation, and instruction/supervision of physical therapy practice according to physician orders. You will work with patients to achieve maximum benefit from their treatment program, re-evaluating and adapting as needed. Additionally, you will instruct and assist patients in using assistive and supportive devices, such as crutches, canes, and prostheses, and confer with physicians and other practitioners to integrate your treatment with other aspects of patient care. You will also evaluate, fit, and adjust prosthetic and orthotic devices and recommend modifications to Orthotists.
Requirements
To be successful in this role, you must have a Bachelor's Degree in Physical Therapy (PT) and a valid Physical Therapist License. You should have excellent communication and interpersonal skills, as well as the ability to work independently and as part of a team. You must also be able to travel frequently to patient homes within designated service areas. In return for your expertise, we offer a competitive salary of $85,000 - $115,000 per year, depending on experience, plus comprehensive benefits, including health insurance, paid time off, and professional development opportunities.
