Acentra Health is seeking a skilled Clinical Assessor to join our team in North Carolina. As a Clinical Assessor, you will play a critical role in determining the level of care required by Medicaid beneficiaries who need assistance with daily living activities.
Key Responsibilities- Conduct comprehensive assessments to determine the level of care needed by beneficiaries
- Ensure that Personal Care Services (PCS) are provided on a needs basis, in quantities appropriate to the beneficiary's unmet needs
- Develop and implement service plans that meet the unique needs of each beneficiary
- Collaborate with healthcare professionals, family members, and caregivers to ensure seamless care coordination
- Participate in training and professional development opportunities to stay up-to-date on best practices and industry developments
- Registered Nurse or LCSW license by the state of North Carolina
- 2+ years of experience in home care or a related field
- Knowledge of North Carolina Medicaid Clinical Coverage Policy (Clinical Policy) 3L and PCS Program Provider Manual
- Ability to work independently and as part of a team
- Excellent communication and interpersonal skills
