We are seeking a skilled Hospice Admission Nurse to join our team at LifeBridge Health. As a Hospice Admission Nurse, you will play a critical role in ensuring that our patients receive high-quality, compassionate care in the comfort of their own homes.
Responsibilities- Assess patients' and families' needs and develop individualized plans of care
- Collaborate with the interdisciplinary team to ensure continuity of care
- Provide professional, comprehensive nursing care to patients and their families
- Communicate effectively with patients, families, and healthcare team members
- Associate's Degree in Nursing
- Minimum of two years of recent medical/surgical nursing experience
- One year of home health experience preferred
- Expertise in performing health assessments
- Bachelor's Degree in Nursing
- CHPN certification
- CPR certification
