We are seeking a highly skilled and compassionate Registered Nurse to join our team as an Ambulatory RN Care Manager at our Lakewood Family Medicine location. As a key member of our healthcare team, you will be responsible for providing exceptional patient care and coordination across the continuum.
Key Responsibilities- Provide patient-centered care and education to promote health and wellness
- Coordinate care transitions and ensure seamless communication with healthcare providers
- Advocate for patients and families, addressing cultural and ethnic sensitivities
- Collaborate with leadership and care teams to identify opportunities for improvement
- Develop and implement plans to promote appropriate resource utilization
- Bachelor's degree in Nursing or equivalent experience
- Current Registered Nurse license in Washington State or Multistate license endorsement
- Master's degree in Nursing preferred
- Certification as a Certified Case Manager (CCMC) or similar required
- Three years of recent clinical hospital/ambulatory experience
