Responsibilities

As a Registered Nurse - Home Health, you'll be the point of contact for all disciplines involved in providing care to patients, overseeing the frequency of visits for the episode. You'll consult with physicians and offices, providing details about patient care, and perform accurate OASIS collection, ensuring medication profiles remain current and lab values are reported to physicians in a timely manner.

You'll also attend weekly case conferences and monthly case manager meetings, ensuring seamless coordination of client care.

Qualifications

To be successful in this role, you'll need:

  • Current licensure as a Registered Nurse (RN) in the state of employment
  • A minimum of one year of clinical experience
  • Demonstrated knowledge and skill in current nursing practice
  • A valid state driver's license and automobile liability insurance
