Care Manager Job Description

LIFEPlan is seeking a dedicated and compassionate Care Manager to join our team. As a Care Manager, you will play a vital role in ensuring our members receive the highest quality care and support.

Key Responsibilities:
  • Provide strong advocacy to ensure members are at the center of determining their desired life goals.
  • Apply empathy, cultural sensitivity, and sound judgment to support members and families.
  • Ensure service standards and compliance are met regarding caseload files and health electronic records.
  • Ensure required OPWDD documentation is complete for member service eligibility.
  • Comply with all standards of conduct and confidentiality as determined by LIFEPlan's Corporate Compliance Regulations, OPWDD, DOH, the Justice Center, and HIPAA.
Requirements:
  • Minimum of a Bachelor's degree in a Health and Human Services field and at least two years of relevant work experience, or experience as a Licensed Registered Nurse with two years of experience, or a Master's degree in a Health and Human Services field with one year of relevant experience.
  • Preferably a strong working knowledge of local community resources.
  • Strong technological skills, including Microsoft Office Suite, Adobe Acrobat, and the ability to learn, access, and navigate web-based systems.
  • Excellent verbal and written communication skills and the ability to actively listen to other points of view, work well with diverse populations, and treat all people with respect and dignity.
  • The ability to thrive in a tele-commuting culture and prioritize work and time appropriately.
