Job Summary

As a Home Hemodialysis Specialist, you will be responsible for providing comprehensive patient care in an independent setting, empowering patients to take control of their health, and working closely with patients, support systems, and care partners to ensure seamless care delivery.

Key Responsibilities:
  • Provide comprehensive patient care in an independent setting, including assessing, diagnosing, and implementing complete care plans.
  • Empower patients to take control of their health by teaching them to administer their own dialysis and promoting self-management.
  • Work closely with patients, support systems, and care partners to ensure seamless care delivery and address any concerns or issues that may arise.
  • Manage patient cases independently, tracking and organizing patient information to ensure timely and effective care delivery.
  • Collaborate with healthcare teams to ensure comprehensive care delivery and address any complex patient needs.
Requirements:
  • Current driver's license and reliable transportation.
  • Associates Degree in Nursing (A.D.N) from an accredited school of nursing, or Bachelor of Science in Nursing (B.S.N), or three-year diploma from an accredited diploma program.
  • Basic computer skills and proficiency in MS Word and Outlook.
Preferred Qualifications:
  • Previous experience in chronic hemodialysis, acute dialysis, peritoneal dialysis, or home hemodialysis nursing.
  • Training/Teaching/Healthcare Education background.
  • Previous case management experience.
  • Familiarity with telehealth technologies.
  • Certified Nephrology Nurse (CNN), Certified Dialysis Nurse (CDN).
