Physical Therapy Care Coordinator

As a key member of our interdisciplinary team, you will provide exceptional care to our participants, enabling them to reach their full potential. Working closely with our clinical and non-clinical staff, you will play a vital role in delivering quality patient-centered care.
Key Responsibilities- Collaborate with physical therapists to develop and implement personalized treatment plans for participants.
- Administer participant programs as established by the physical therapist, ensuring seamless execution of care.
- Assist in obtaining vital signs, range of motion, and strength testing to inform care decisions.
- Coordinate with other rehabilitation team members to implement activities and programs tailored to individual needs.
- Manage scheduling, clinical visits, and billing processes efficiently.
- Deliver and document physical therapy interventions according to established care plans, promoting active and passive exercise, muscle re-education, gait training, and functional training.
- Participate in end-of-life care coordination and support services.
- A minimum of an associate's degree in Physical Therapy Assistant from an accredited program.
- Valid Physical Therapy Assistant license or proof of eligibility in the state practicing.
- Two years of experience as a Physical Therapy Assistant in a clinical setting with a frail or elderly population is preferred.

Estimated Salary Range: $33.85 - $43.04 USD per hour, plus bonus and equity.
