Responsibilities

  • Care Planning: Collaborate with clients, families, and healthcare professionals to create individualized care plans that address each client's unique needs and goals.
  • Service Coordination: Coordinate services and resources to ensure timely and effective delivery of care, including referrals to community agencies and providers.
  • Clinical Expertise: Utilize your nursing expertise to provide direct care and guidance to clients and caregivers, addressing medical questions and concerns as needed.
  • Communication: Maintain regular communication with clients, families, and healthcare professionals to ensure open exchange of information and collaboration.
Requirements
  • Licensure: Current licensure as a Registered Nurse in the State of Minnesota required.
  • Experience: Minimum two years of experience in a similar position and care environment preferred.
  • Certifications: Current CPR certification and maintenance of certification required.
  • Educational Background: Associate's or Bachelor's degree in Nursing from an accredited program.
Estimated Salary Range:

$65,000 - $80,000 per year, depending on qualifications and experience.
