About the Role

We are seeking a skilled Licensed Clinical Social Worker to join our team in Waukesha County, WI. As a key member of our multi-disciplinary team, you will provide social work intervention, counseling, and crisis intervention to patients/clients in need.

Responsibilities
  • Assess patient's psychosocial needs and develop plans to meet established goals
  • Counsel individuals and families in crisis, addressing issues such as poverty, loss, and illness
  • Collaborate with all disciplines to plan and evaluate team goals for each patient/client
  • Facilitate patient/client and family referrals to community agencies when appropriate
  • Provide financial counseling to achieve treatment goals
  • Respond to crisis situations, including domestic violence and child abuse
  • May be required to provide witness services and evidence in court cases
  • Visit schools to assess academic concerns
  • Work with government agencies to help clients apply for Medicare and Medicaid
  • Arrange home visits to evaluate patient/client health, home environment, and family interaction/support
Requirements
  • Master's degree in Social Work from a school of social work accredited by the Council on Social Work Education
  • Active license as an LCSW in state of practice
  • Minimum of one year of social work experience preferred
  • Complies with all relevant professional standards of practice
  • Participation and completion of Amergis' Competency program when applicable
  • Current CPR if applicable
  • TB questionnaire, PPD or chest x-ray if applicable
  • Current Health certificate (per contract or state regulation)
  • Must meet all federal, state, and local requirements
  • Successful completion of new hire training as applicable to job site
  • Understand patient confidentiality and HIPAA requirements
  • Ability to effectively elicit/provide information to and from appropriate individuals via strong communication skills
  • Computer proficiency required
  • Must be at least 18 years of age
