Job Summary

We are seeking a highly skilled and experienced LPN Nurse Supervisor to join our team at Christian Extended Care & Rehabilitation. As a key member of our nursing leadership team, you will be responsible for overseeing the delivery of high-quality patient care and ensuring the well-being of our residents.

Key Responsibilities
  • Staff Management: Assign staff according to policies and ensure equal distribution of assigned duties.
  • Teamwork and Communication: Support teamwork and encourage staff to provide resident care properly and in an acceptable time frame.
  • Assessment and Reporting: Complete rounds to assess residents and resident care, identify and report concerns and needs to appropriate staff members in a professional and timely manner.
  • Documentation and Reporting: Receive report from assigned charge nurses in a timely manner and complete a shift report for the Director of Nursing.
  • Discipline and Performance Management: Appropriately manage staff and discipline staff members as necessary.
  • Employee Development: Assist employees in resolving unsatisfactory performance and behavior issues in a positive and constructive manner.
  • Quality Improvement: Empower staff members by promoting responsibility, accountability, and autonomy to enhance resident care.
  • Standards of Care: Ensure standards of patient care are utilized and delegate responsibilities as defined by policy or necessary to ensure appropriate resident care.
  • Care Planning: From resident assessment, review and revise the care plan needs, problems, and goal changes with the Charge Nurses.
  • Nursing Interventions: Identify necessary and appropriate nursing interventions pertinent to the plan of care.
  • Education and Training: Implement and document all resident/family teaching relevant to health care needs and ensure understanding of the teaching.
  • Direct Patient Care: Actively participate in the direct delivery of patient care.
  • Environment of Care: Ensure that a safe, comfortable, clean environment is maintained.
  • Reporting and Communication: Complete monthly QAPI, daily nursing labor forms for Administration, and monitors pharmacy recommendations monthly.
  • Meetings and Collaboration: Attend Team Leader meetings twice per month and CNA meetings monthly.
  • Emergency Procedures: Maintain working knowledge of emergency procedures and demonstrate awareness of safety issues relevant to patient and employee safety.
  • Crisis Management: Handle crisis situations in a prompt, precise, and professional manner.
  • Facility Operations: Coordinate activities and assignments throughout the facility to ensure appropriate functioning of each neighborhood.
  • Problem-Solving: Analyze problems, offer recommendations, and initiate problem resolution in a professional and timely manner.
  • Incident Investigation: Investigate employee concerns or incidents and take the appropriate action.
Requirements
  • Current LPN licensure in the state of employment
  • Two years of long-term care supervisory experience preferred
  • Three to five years of long-term care experience or similar nursing experience
  • Excellent communication skills to interact verbally with residents, visitors, and staff
  • Ability to prioritize, multi-task, manage time, and organize daily schedule to meet productivity standards
  • Ability to stand and walk for long periods of time
  • Ability to lift heavy objects using proper lifting techniques and with assistance as necessary
  • Ability to transfer and move residents who vary in ability to assist weighing 200+lbs using proper lifting techniques and with assistance as necessary
  • Ability to push and pull equipment weighing in excess of 40lbs using proper techniques and with assistance as necessary
  • Manual dexterity, coordination, and skillful use of hands when working with residents and equipment
  • Ability to perform necessary documentation
