Physical Therapist Home Health Full Time

We are seeking a skilled Physical Therapist to join our Home Health team. As a Physical Therapist, you will play a vital role in helping patients recover from injuries, illnesses, and functional disabilities.
Key Responsibilities- Plan and administer prescribed skilled physical therapy treatment and training for patients.
- Test and screen patients' physical strengths to assist physicians in evaluating their level of function.
- Develop and implement a conditioning/rehabilitation program consistent with the physician's Plan of Treatment.
- Confer with physicians and clinical team members to obtain additional patient information and assist in developing, implementing, and revising the therapy treatment program.
- Provide Physical Therapy Assistants and Home Health Aide staff with written instructions/care plans that reflect the current plan of care.
- Degree from an accredited Physical Therapy Program.
- Minimum of one year physical therapy experience preferred.
- Current and unrestricted Physical Therapy license.
- Current CPR certification.
- Strong organizational and communication skills.
