Penn State Health is seeking a dedicated Healthcare Professional to join our team as a Stroke and Sepsis Program Coordinator.

About the Role

We are looking for an experienced individual to administer and coordinate all multidisciplinary activities associated with the Stroke and Sepsis programs. The ideal candidate will work collaboratively with Medical Directors, coordinators at other sites, and nurse leaders to ensure the success of these programs.

The successful candidate will possess strong knowledge and skills to provide care for adult and geriatric patients participating in these programs. Key responsibilities include:

  • Managing data related to the programs
  • Providing necessary support to ensure program-related activities are carried out
  • Coordinating team efforts, chart reviews, real-time evaluations, and documentation
  • Delivering bedside education for patients and staff, facilitating support groups, and organizing educational events
Requirements

To be considered for this role, applicants must have a minimum of three years of related clinical experience. A State RN License is required, and certifications in related areas are preferred. Experience in performance improvement and program management or research backgrounds are also highly valued.
