Job Title: LPN

At Lakeshore Community Services, Inc., we are seeking a highly skilled and dedicated Licensed Practical Nurse (LPN) to join our team.

Job Summary:

The LPN will be responsible for providing high-quality nursing care to our consumers, conducting quarterly assessments, and maintaining accurate records. The ideal candidate will have excellent communication skills, a strong work ethic, and the ability to work effectively in a team environment.

Key Responsibilities:

  • Conduct quarterly nursing assessments and maintain accurate records.
  • Provide nursing care to consumers with complex medical or psychiatric needs.
  • Respond to consumer-specific needs, including teaching and support of medical conditions.
  • Meet with the treatment team and program staff to discuss consumer progress.
  • Maintain a consumer record system that meets current regulations and HIPAA requirements.
  • Communicate with staff regarding consumer needs and changes.
  • Coordinate training for staff and consumers on consumer needs.
  • Identify and respond to medication variances.
  • Work with the waiver RN to ensure seamless care.
  • Assist with obtaining and following up on doctor's clinic refills.
  • Review and approve medical reports in Evolve.
  • Offer teaching and support related to medication changes.
  • Participate in discharge planning and obtain discharge papers and medical records.
  • Provide follow-up care to consumers after discharge.
  • Maintain effective communication within the program and with external programs.
  • Provide on-call support during non-business hours.
  • Attend in-service training sessions as required.

Requirements:

  • LPN licensure (required)
  • Excellent communication and interpersonal skills
  • Ability to work effectively in a team environment
  • Strong work ethic and attention to detail
