Hospice Palliative Care Nurse Practitioner

This position requires a skilled Advanced Practice Registered Nurse to work in hospice and palliative care settings. As a Nurse Practitioner-Hospice-Palliative, you will provide high-quality patient care through assessment, planning, implementation, and evaluation of patients' needs.
The ideal candidate will possess strong communication skills, ability to work independently, and experience in providing nursing care to patients with serious and terminal illnesses. You will be responsible for collaborating with healthcare teams, performing diagnostic procedures, prescribing medications, and educating patients and families about health status and alternatives for care.
Responsibilities
- Assess patients' health status by taking medical histories, performing physical examinations, and interpreting lab results.
- Develop and implement individualized care plans to address patients' needs and goals.
- Prescribe medications and treatments based on patients' diagnoses and conditions.
- Counsel patients and families about health status, treatment options, and disease management.
- Collaborate with healthcare teams to ensure seamless transitions of care.
Requirements
- Masters prepared registered nurse (RN) with current licensure to practice in Nevada.
- Two years minimum experience as an RN required; six months clinical experience preferred as a Nurse Practitioner.
- Valid Advanced Practice Registered Nurse license with the State of Nevada, Controlled substance License to Prescribe issued by the State of Nevada, and Controlled Substance Registration Certificate issued by the DEA.
- American Academy of Nurse Practitioners Certification Board (AANPCB) certification in Family Nurse Practitioner (FNP), Adult-Gerontology Primary Care Nurse Practitioner (A-GNP), or Emergency Nurse Practitioner (ENP).
