We're seeking a compassionate and dedicated Home Health Aide to provide exceptional care to our clients in the Queens area. As a Home Health Aide with CareConnect, you will play a vital role in ensuring our clients receive the highest level of care and support in the comfort of their own homes.

Key Responsibilities:

  • Provide personalized care to patients, including bathing, dressing, and grooming, to promote their physical and emotional well-being.
  • Assist with mobility and transfers, ensuring the safety and dignity of patients.
  • Administer medications as directed by healthcare professionals, maintaining accurate records and reporting any concerns.
  • Monitor vital signs and report any changes or concerns to the appropriate medical personnel, ensuring timely interventions.
  • Provide emotional support and companionship to patients and their families, fostering a sense of connection and community.
  • Maintain a clean and safe environment for patients, adhering to infection control protocols and ensuring a healthy living space.

Requirements:

  • Certified Personal Care Assistant (PCA) or Certified Home Health Aide (HHA) certification.
  • Understanding of HIPAA regulations and the importance of patient confidentiality.
  • Ability to cook simple meals and perform light housekeeping duties as needed, promoting independence and dignity.
  • Satisfactory completion of New York State Health Department-approved HHA Training Program or equivalency.
  • Demonstrated adequate literacy and simple arithmetic skills, ensuring effective communication and record-keeping.
  • Must be at least 18 years old, with a strong commitment to providing high-quality care.
