Nurse Practitioner

SAC Health is seeking a highly skilled and compassionate Registered Nurse to join our team. As a Registered Nurse, you will play a vital role in providing exceptional patient care and contributing to the success of our organization.
Job Summary:The Registered Nurse will work under the supervision of the RN Manager and will be responsible for providing nursing care to patients in an outpatient setting. This includes assessing patient needs, developing and implementing individualized care plans, and evaluating patient outcomes. The ideal candidate will possess excellent communication and interpersonal skills, as well as the ability to work effectively in a fast-paced environment.
Responsibilities:- Provide high-quality patient care, including assessment, planning, implementation, and evaluation of care.
- Develop and implement individualized care plans that meet the unique needs of each patient.
- Collaborate with other healthcare professionals to ensure seamless care coordination.
- Communicate effectively with patients, families, and healthcare teams to ensure optimal patient outcomes.
- Participate in quality improvement initiatives to enhance patient care and safety.
- Maintain accurate and up-to-date patient records, including documentation of care and treatment.
- Perform other duties as assigned by the RN Manager or other healthcare professionals.
- Current and unrestricted California Registered Nurse license.
- Minimum two (2) years of experience as a Registered Nurse, preferably in an outpatient setting.
- Basic Life Support (BLS) certification required.
- Valid California Driver's License and liability car insurance.
- Ability to work effectively in a fast-paced environment and prioritize tasks to meet deadlines.
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
- Ability to maintain accurate and up-to-date patient records, including documentation of care and treatment.
