Job Summary:

  • Deliver specialized evaluations and evidence-based treatment plans tailored to each patient's specific needs
  • Collaborate with patients, clinical teammates, and organization members to achieve shared goals

Responsibilities:

  1. Conduct comprehensive patient assessments and develop personalized treatment plans
  2. Engage with patients and their families to ensure informed decision-making and optimal outcomes
  3. Work closely with multidisciplinary teams to provide seamless care coordination
  4. Maintain accurate and detailed records of patient interactions and progress

Requirements:

  • Graduate of an accredited Physical Therapy Program
  • Current or pending state licensure as a Physical Therapist
