SSM Health is seeking a dedicated and compassionate Hospice RN to provide end-of-life care to patients in their homes and facilities. As a Hospice RN, you will be responsible for implementing and monitoring patient care plans, coordinating with the healthcare team, and providing emotional and spiritual support to patients and their families.
Key Responsibilities:
- Implement and monitor patient care plans in accordance with established policies and procedures
- Coordinate with the healthcare team to ensure well-coordinated patient care
- Assess and coordinate patient's discharge planning needs
- Provide emotional and spiritual support to patients and their families
Requirements:
- Graduate of an accredited school of nursing
- No experience required
